Résumé

Introduction : La maladie de Rosai-Dorfman (MRD) est une
histiocytose rare de présentation clinique polymorphe. Elle
intéresse surtout les ganglions lymphatiques du cou. En
revanche, l’atteinte multifocale est rarement décrite. Nous
rapportons une observation inédite de maladie de Rosai-
Dorfman particulière par son atteinte multifocale associant
des localisations ganglionnaires, hépatosplénique, rénale et
osseuses chez un jeune patient.

Observation : Un homme âgé de 24 ans est hospitalisé pour
exploration d’adénopathies cervicales de 2 cm de diamètre
sans signes inflammatoires locaux. La biopsie ganglionnaire
a montré un aspect cadrant avec une histiocytose de Rosai-
Dorfman. La TDM thoraco-abdomino-pelvienne (TAP) montrait
une hépato-splénomégalie homogène. Un traitement par
corticothérapie (1 mg/kg par jour d’équivalent de prednisone)
a été instauré. L’évolution initiale était favorable mais le patient
a été perdu de vue pendant 7 ans et a arrêté la corticothérapie.
L’évolution était marquée par la ré-augmentation progressive
de la taille des adénopathies cervicales et sus claviculaires.
L’étude histologique d’un ganglion cervical était compatible
avec l’histiocytose sinusale de Rosai-Dorfman. La TDM TAP
montrait de multiples adénomégalies cervicales et sus
claviculaires, une hépato-splénomégalie homogène, une
masse para-pyélique rénale droite de densité tissulaire et une
atteinte ostéolytique diffuse du squelette axial (sternum, ailes
iliaques, clavicules, scapulas, côtes, 2 têtes humérales). La
corticothérapie était indiquée devant le tableau systémique
avec atteinte multiviscérale. Le patient est à nouveau perdu
de vue.

Conclusion : La MRD est une histiocytose rare
d’individualisation récente. Le caractère ubiquitaire de la
maladie, la non-spécificité de sa présentation clinique et
radiologique posent des défis diagnostiques.

Abstract

Introduction: Rosai-Dorfman disease (RDD) is a rare
histiocytosis with a polymorphic clinical presentation. It
mainly affects the lymph nodes in the neck. Multifocal
involvement is rarely described. We report a novel
observation of a particular Rosai-Dorfman disease with
multifocal involvement of lymph node, hepatosplenic,
kidney and bone locations in a young patient.

Observation: A 24-year-old man was hospitalized for
exploration of cervical polyadenopathies of 2 cm in diameter
without local inflammatory signs. Node biopsy was
compatible with Rosai-Dorfman histiocyotosis. Thoracoabdomino-
pelvic CT scan (TAP) showed homogeneous
hepato-splenomegaly. Treatment with corticosteroid
therapy (1mg/kg per day prednisone equivalent) was
instituted. The initial evolution was favourable but the
patient was lost to follow-up for 7 years and stopped
corticotherapy. The evolution was marked by progressive
increase in the size of the cervical and supraclavicular
nodes. The histological study of a cervical node was
compatible with Rosai-Dorfman’s sinus histiocytosis. The
TAP CT scan showed multiple cervical and supraclavicular
nodes, homogeneous hepato-splenomegaly, right renal
para-pyelic mass with high tissue density and diffuse
osteolytic damage to the axial skeleton (sternum, iliac
wings, clavicles, scapulas, ribs, both humeral heads).
Corticotherapy was indicated given the systemic form with
multi-visceral involvement. The patient was lost from follow
up.

Conclusion: MRD is a rare histiocytosis of recent
indivualisation. The ubiquitous nature of the disease
and the non-specificity of its clinical and radiological
presentation pose diagnostic challenges.
