Importance des shunt lympho-veineux chez des patientes avec un oedème des membres inférieurs : à propos de leur démonstration lymphocintigraphique dans 5 cas
Importance of the lympho-venous shunt in patients with lower limb lymphedemas: lymphoscintigraphic demonstrations in 5 cases
L’existence de communications lymphatico-veineuses a été décrite il y a plus de cinquante ans et leur présence a été mise en évidence à l’aide de radioisotopes et dans le cadre de lymphangiographies radiologiques chez des malades présentant un cancer ou un lymphoedème. Nous rapportons ici les imageries lymphoscintigraphiques de 4 femme et un homme avec un oedème des membres inferieurs secondaire à une chirurgie pour cancer (3 ovaires, 1 col utérin, 1 prostate) où la présence d’une communication lymphatico-veineuse pouvait être suspectée ou a été démontrée. En rapportant ces cas, le but de cet article est (1) de rappeler au corps médical l’existence de ces shunts entre le système lymphatique et la circulation systémique avec ses implications potentielles, (2) d’attirer l’attention des spécialistes en médecine nucléaire pratiquant leurs investigations dans le cadre de lymphoedèmes des membres inférieurs sur les images évoquant ces situations mais aussi (3) de leur proposer la réalisation de la phase 4 pour pouvoir les mettre en évidence.
The existence of lymphatico-venous shunts was described more than fifty years ago in the framework of the real ization of diagnostic radiological lymphangiographies and of the therapeutic intralymphatic injections of radio-labeled materials in patients presenting a cancer or a lymphedemas. We report lymphoscintigraphic observations of 4 women and 1 men with a secondary oedema of lower limb to a surgery for cancer (3 ovarian, 1 uterine cervix, 1 prostate) where the presence of lymphatico-venous shunt could be suspected or was demonstrated. By reporting these cases, the aim of the present paper is (1) to remind to medical profession the existence of these shunts between the lymphatic system and the systemic circulation with its potential implications, (2) to draw the attention of the specialist in nuclear medicine practicing their investigations within the framework of lymphedemas lower limbs on the images evoking these situations but also (3) to propose them the realization of the phase 4 to be able to highlight them.
