Is evidence-based medicine applicable in geriatrics ?
A l’occasion de la présentation d’un cas clinique, nous discuterons des barrières à l’implémentation de la médecine factuelle pour le patient âgé en médecine générale. Nous aborderons le concept de fragilité, les difficultés de la prise en charge des patients présentant de multiples conditions chroniques, les prescriptions potentiellement inappropriées et un essai de définition du patient gériatrique. Le vieillissement de la population apporte un concept nouveau : « les syndromes gériatriques ». L’avènement des syndromes gériatriques constitue « une nouvelle ère de la médecine » nécessitant le développement de nouveaux modèles d’approches au niveaux diagnostique, thérapeutique et de recherche.
During the presentation of a clinical case, we will discuss the barriers to the implementation of evidence-based medicine for the geriatric patient in general practice. We will discuss the concept of frailty, the difficulties of the management of patients with multiple chronic conditions, potentially inappropriate prescriptions, and an attempt to define the geriatric patient. The aging of the population brings a new concept: “the geriatric syndrome”. The advent of geriatric syndromes constitutes «a new era of medicine» requiring the development of new models of approaches at the diagnostic, therapeutic and research levels.
