La sténose aortique calcifiée du sujet âgé est une pathologie dont la fréquence est en constante augmentation en raison du vieillissement de la population. Dans cet article, nous tenterons de répondre aux questions que peut se poser le médecin traitant dans la prise en charge de cette pathologie, longtemps asymptomatique. Nous nous intéresserons au diagnostic de la pathologie. Nous évoquerons les différentes options thérapeutiques, médicale, chirurgicale, en incluant les nouvelles techniques de remplacement valvulaire par cathétérisme. Nous préciserons les éléments qui déterminent le choix du traitement et le moment opportun d’intervenir. Because of population ageing, the frequency of calcified aortic stenosis in the elderly is constantly rising. In this article we will attempt to answer the questions a General Practitioner might ask himself when treating this disease that remains symptomless for quite some time. We will discuss the diagnosis of this pathology and the various medical and surgical therapeutic options including the newly developed technique of trans catheter aortic valve replacement. We will list the elements determining treatment choice and optimal timing for intervention.
