Les vertiges représentent 5 % des consultations des généralistes. Il s’agit donc de plaintes très fréquentes souvent d’origine vestibulaire. Il est expliqué comment à partir de l’évocation des mots “ vertige ou instabilité ” une anamnèse ciblée amènera à un examen clinique précis et au diagnostic des pathologies O.R.L. principalement rencontrées. La sémiologie, l’examen otoneurologique, les étiologies et le ou les traitement(s) seront abordés en détail. Volontairement, les examens complémentaires réalisés dans le service O.R.L. ne sont pas expliqués pour rester en phase face aux possibilités de diagnostic au cabinet ou au lit du patient. Les pathologies essentiellement traitées sont : le vertige paroxystique bénin de position, la neuronite vestibulaire, la maladie de Ménière et les vertiges de la personne âgée. Cet article a pour but d’identifier plus facilement les diverses pathologies vestibulaires et de les référer si nécessaire dans le service adéquat. Rev Med Brux 2013 ; 34 : 251-6
Vertigoes represent 5 % of GP consultations. We thus often deal with complaints from vestibular origin. The starting point of this text is the word “ vertigo or instability ” as well as a targeted anamnesis allowing a general approach leading to a precise clinical examination and thus to a diagnosis of most commonly encountered E.N.T. pathologies. The anamnesis, the oto-neurological test and the treatment(s) will be further developed. Willingly the complementary tests performed in the E.N.T. unit have been skipped and we will keep the focus on the anamnesis and the clinical examination that can be can performed in your practice or at patient’s house. The main overviewed pathologies will be : paroxysmal positional vertigo, vestibular hypofunction, Meniere’s disease and dizziness in the elderly.
