Simple and complex post-traumatic disorders: management and treatment options
La clinique du psychotrauma est caractérisée par des
manifestations cliniques très variées et peu spécifiques qui
peuvent rendre le diagnostic difficile, avec des patients à
risque de se retrouver dans des situations que nous pourrions
qualifier « d’errance diagnostique ».
Les médecins généralistes seront souvent les premiers
à accueillir ces patients dans une première démarche de
demande de soin souvent organisée autour d’une variété de
symptômes, expressions psychosomatiques et comorbidités
physiques de la détresse psychique qui caractérise cette
population.
Par contre, envisager ce diagnostic en présence de ces
symptômes permet d’adapter sa prise en charge, faciliter
l’orientation thérapeutique et favoriser l’évolution clinique de
ces patients.
Je présenterai dans cet article deux cas cliniques représentatifs
de traumatisme simple et de traumatisme complexe pour
souligner les différences d’étiologie, d’expressions cliniques
et d’options thérapeutiques de chacun.
The clinic of psychotrauma is characterized by several
varied and unspecific clinical manifestations that can
make the diagnosis difficult, with patients at risk of
finding themselves in a process that we could describe as
«diagnostic wandering».
General practitioners will often be the first to welcome
these patients for an initial request for care. This is often
performed by considering the broad range a of symptoms,
psychosomatic expressions, and comorbidities of the
psychological distress, which characterize this population.
On the other hand, considering this diagnosis in the
presence of certain symptoms makes it possible to adapt
management, facilitate therapeutic orientation and
promote the clinical evolution of these patients.
In this article, I will present two representative clinical cases
of simple and complex trauma to highlight the differences
in etiology, clinical expressions, and therapeutic options of
each.
