Fibromyalgia treatment: a narrative review
Introduction: Fibromyalgia is characterised by widespread pain, fatigue and sleep disorders. Last recommendations for fibromyalgia treatment have been published in 2017. In the first intention, physiotherapy and psychotherapy are recommended. Drugs are only used if physiotherapy and psychotherapy are insufficient. The goal of this work is to review recent treatments proposed to manage fibromyalgia. Material et methods: The databases searched were Pubmed and Cochrane using the following keywords: “Fibromyalgia”, “Treatment” and “Guidelines”. Meta analysis and systematic reviews from 2017 to 2025 are included according to the research question and the main outcome. The data recorded are: the population and the number of participants, the intervention, the control group, the results and the principal conclusions.
Results: Among 382 articles, 28 reviews have been included in this work according to our selection criteria. Physiotherapy (aerobic exercises combined with resistance exercises) is recommended. Psychoeducation is efficient. Combined with physiotherapy and psychotherapy, pharmacologic treatments such as Duloxetine and Gabapentine could be secondly added according to associated clinical symptoms. Up to now, transcranial stimulation and cannabinoids are hopeful but cannot be recommended.
Conclusions: First and foremost, fibromyalgia needs a tailored multidisciplinary support pairing physiotherapy with psychotherapy. A second-line pharmacologic treatment could be discussed with the patient. Other techniques such as transcranial stimulation and cannabinoids are promising but further studies are needed to recommend these interventions.
