Dépistage du risque de déclin fonctionnel par une équipe gériatrique mobile au sein d’un hôpital général (accès libre)
Screening of the risk of functional decline performed by an inpatient geriatric consultation team in a general hospital (free access)
L’Equipe Gériatrique Mobile (EGM) fait partie du Programme de Soins Gériatriques et a pour but de mettre l’expertise gériatrique à la disposition d’autres professionnels pour des patients âgés hospitalisés hors unités de gériatrie. Notre Institution dispose depuis 2008 d’une EGM. L’objectif de cette étude rétrospective est de décrire la population de patients âgés de 75 ans et plus et évalués par l’EGM entre le 1er octobre 2009 et le 30 septembre 2011. Le risque de déclin fonctionnel est évalué par le score Identification of Senior At Risk (ISAR), réalisé endéans les 48 premières heures après l’admission. On détermine également le lieu de vie, la destination après la sortie, le Mini Mental State Examination (MMSE) et la Geriatric Depression Scale (GDS). En deux ans, 1.568 patients >= 75 ans ont été évalués par l’EGM (âge moyen : 82,5 ans ; 60,7 % de femmes). Parmi eux, 833 patients à haut risque de déclin fonctionnel (ISAR >= 3) ont été identifiés. La majorité des sujets à haut risque de déclin fonctionnel (78 %) vivaient à domicile et 58,7 % y sont retournés après l’hospitalisation. Une suspicion de dépression et d’atteinte cognitive était présente parmi respectivement 41 % et 59 % de sujets à haut risque. Parmi les 128 patients admis suite à une chute, la plupart vivaient à domicile et avaient un score ISAR >= 3. L’EGM a permis d’identifier un grand nombre de patients >= 75 ans vivant à domicile et présentant un haut risque de déclin fonctionnel ainsi que des syndromes gériatriques, ce qui confirme la nécessité des procédures de dépistage. La plupart des patients admis après une chute ont un score ISAR >= 3 et nécessitent une évaluation gériatrique globale. Rev Med Brux 2013 ; 34 : 462-8
The Mobile Geriatric Team (MGT) is part of the Geriatric Care Program and aims to provide interdisciplinary geriatric expertise to other professionals for old patients hospitalized outside geriatric department. Our hospital has a MGT since 2008. Our objective is to retrospectively describe the population of patients of 75 years and older hospitalized outside the geriatric ward and screened for the risk of functional decline by the MGT between 1 October 2009 and 30 September 2011. We recorded the risk of functional decline, as indicated by the Identification of Senior At Risk score (ISAR) performed within 48 h after admission, place of living, discharge destination, Mini Mental State Examination (MMSE) and Geriatric Depression Scale (GDS) scores. In two years, 1.568 patients >= 75 Y were screened with the ISAR score (mean age 82.5 Y, 60.7 % of women). We identified 833 patients with a high-risk of functional decline (ISAR >= 3). The majority of high-risk subjects (78 %) were living at home before hospitalization and 58.7 % returned home after discharge. Depression and cognitive impairment were identified among respectively 41 % and 59 % of high-risk subjects. Only 128 patients were admitted for fall. Most of the faller patients were living at home prior hospitalization and had an ISAR score >= 3. The MGT allowed identifying many patients >= 75 Y living at home and presenting with high-risk of functional decline and geriatric syndromes, confirming that good screening procedures are necessary to optimize management of hospitalized olders.
