Paraneoplastic hypercortisolism : case report and review of available therapeutic options
L’hypercorticisme paranéoplasique est une entité rare mais qui peut poser des problèmes importants de diagnostic et de prise en charge, ce dû à la symptomatologie invalidante, aux désordres hydroélectrolytiques et au taux d’infection élevé avec comme conséquence un pronostic vital amoindri. Chez ces patients, le traitement principal repose sur le contrôle de la maladie néoplasique par un traitement local (chirurgie ou radiothérapie) ou par une thérapie systémique. Tout ceci implique une stabilisation de l’état clinique provoqué par l’hypercorticisme afin que les traitements visant la pathologie cancéreuse puissent être délivrés dans des conditions de sûreté. A travers un cas clinique, nous discutons les options thérapeutiques actuelles.
Paraneoplastic hypercortisolism is a rare clinical entity that may cause diagnostic and treatment problems, in relation to i ts disabling symptomatology, hydroelectrolytical disorders and the high rate of infection which induce a poor prognosis. Here we report the case of a 51 years old female diagnosed with a small cells lung cancer responsible for paraneoplastic hypercortisolism. Her symptomatology includes a cervicofacial edema, a thigh pain, a walk instability, a muscular weakness, a mMRC grade III dyspnea, a polyuria, a polydipsia and foot paresthesis of recent appearance. In those patients, the main treatment is based on the control of the neoplastic disease responsible for hypercortisolism syndrome, either by local treatment (surgery or radiotherapy) or by systemic therapy. Before, it requires the stabilization of the clinical status caused by the hypercortisolism context so that the treatments that target the cancerous lesion can be delivered as safe as possible. Through this clinical case we discuss the current therapeutic options to achieve this end.
