Les recommandations 2018 des sociétés européennes de cardiologie et d’hypertension sur la prise en charge de l’hypertension artérielle proposent un nouveau schéma thérapeutique. Il est basé sur un concept précis de seuil tensionnel à partir duquel traiter et d’objectifs tensionnels qui sont fonction du niveau de pression artérielle de départ, de l’âge et des comorbidités. Le traitement médicamenteux proposé comprend d’emblée l’administration d’une combinaison de deux molécules dans un seul comprimé. Là où trois antihypertenseurs doivent être utilisés les recommandations consacrent l’usage d’une bloqueur du système rénine angiotensine combiné à antagoniste calcique et un diurétique thiazidé.
The new 2018 guidelines for the management of arterial hypertension proposed by the European societies of cardiology and hypertension propose a completely new therapeutic strategy based on clearly defined threshold for treatment and targets. Therapy will be based on initial blood pressure levels, patient age and comorbidities. Initial therapy should now be based on dual combination therapy in a single pill. When three antihypertensives will have to be used their combination should be a blocker of the renin angiotensin system, a calcium antagonist and a thiazide diuretic.
