Une céphalée en coup de tonnerre révélatrice d’une apoplexie hypophysaire

L’apoplexie hypohysaire est une entité clinique rare mais importante à reconnaitre puisqu’elle peut mettre en jeu le pronostic vital. Nous rapportons le cas d’un patient de 56 ans aux antécédents d’hypertension et d’hyperlipidémie se présentant avec des céphalées intenses, en coup de tonnerre, inhabituelles, accompagnées de vomissements. Le bilan iconographique comprenant une résonance magnétique cérébrale et un bilan hormonal mettront en évidence une apoplexie hypophysaire. L’analyse des axes thyréotropes, corticotropes et gonadotropes confirmeront l’insuffisance hypophysaire antérieure. L’évolution sera favorable avec un traitement conservateur et l’instauration d’une hormonothérapie substitutive.
Pituitary apoplexy is a rare but important clinical entity to recognize because of its potentially lifethreatening evolution. We report the case of a 56-year-old patient with a history of hypertension and hyperlipidemia presenting with severe, thunderclap, unusual headaches accompanied by vomiting. The iconographic assessment including a cerebral magnetic resonance and a hormonal evaluation highlight a pituitary apoplexy. Analysis of thyrotropic, corticotropic and gonadotropic axes confirm anterior pituitary insufficiency. The evolution was favorable with conservative treatment and the introduction of hormone replacement therapy.
